Congestive Heart Failure/Heart Health Education

Heart health is a major concern for our region, most significantly Congestive Heart Failure (CHF). Our clinical teams use the latest most effective evidence-based methods to care for cardiac patients. Working closely with the patient, their physician and family, each clinician directs a personalized plan of care designed with the individual in mind to both treat and educate the patient and family to manage their cardiac issues.

  • CHF readmission rate: Readmission into the hospital is a major concern for CHF patients. The Home Health Center of Thibodaux Regional has one of the lowest hospital readmission rates in the region for CHF patients.

Wound Care

Managed and supervised by a Certified Wound Care Specialist, Thibodaux Regional Home Health Center can care for the most complex wounds. With years of experience, the clinical staff administers the highest quality, most advanced wound care available.

Medication Management

Medication compliance and management is an integral part of any patient’s path to recovery and future health. And education is a key component to its success. Registered Nurses educate the patients on their individual medications, including dosage, side effects, and potential adverse interactions with other medications along with the safe management of their medication regimen.

Specialty Orthopedic Program

With the increased advancements in orthopedic procedures, total knee and hip replacements are more prevalent than ever. The Orthopedic Program (OP) is a home-based rehabilitation program designed to educate the patient and treat them using a proven, comprehensive therapy program after surgery. The physical-therapy driven OP has been extremely successful over the years in getting our patients back to their active, independent life both quickly and safely.

Fall Prevention/Balance Improvement

Independence and safety go hand-in-hand in the home. Whether an elderly patient or a younger individual who may be struggling with balance issues, fall prevention, and balance improvement are the keys to staying independent. Our physical therapists and other clinicians work closely with patients to ensure safe, confident ambulation and transfers. Both patients and caregivers are trained in correct ambulation, balance and transferring, and maintaining a safe home environment.

COPD

Chronic Obstructive Pulmonary Disease (COPD) can range from an agitating cough to difficulty breathing or shortness of breath. Through proper treatment and disease management, patients can take control of this disease and not have their lives defined by it. In partnership with their physician, a specialized plan of care will be developed and implemented to give the patient the clinical care and the tools they need to manage COPD. Manage it both for the short-term and to provide them with the knowledge to maintain a productive, full life with COPD.

Pneumonia

Pneumonia can be a serious concern if you're 65 or older or have a chronic illness or weakened immune system. It can range in severity from mild to life-threatening. Quick treatment by your physician and appropriate follow-up and monitoring in the home setting can drastically reduce the risk of complications and worsening of pneumonia. Registered Nurses will closely monitor your condition in your home and report back to your physician any abnormalities or conditions that are present to allow for the disease to worsen. Consistent care is the key to complete, successful recovery.

Diabetes is a common disease, yet individuals need unique care. Learning as much as possible about the latest medical therapies and approaches, as well as making healthy lifestyle choices, will greatly assist the patient and their families in dealing with Diabetes.
