Education and Resources for Managing Diabetes
The Diabetes Self-Management Program of Thibodaux Regional offers comprehensive
diabetes services, instruction and support to hundreds of inpatients and
outpatients, as well as their families and loved ones.
Diabetes is a leading cause of disability and greatly increases the risk
of death from heart attack and stroke. As a result of advanced medical
treatments and self-management education programs, people with diabetes
may be spared many complications and premature death. Education is the
cornerstone of diabetes management. The program focuses on each patient’s
individual needs for learning and implementing strategies that will affect
lifestyle changes for a healthier life.
Diabetes Self-Management Services
Diabetes Self-Management Services at the Wellness Education Center offers
comprehensive services and instruction to inpatients and outpatients as
well as their families and loved ones. Instruction is offered to those
with Type I, Type II or gestational diabetes.
What Services Does Diabetes Self-Management Services Provide?
Diabetes Self-Management Services offers a number of helpful services to
patients including:
- Diet Management: Balancing what and when you eat
- Exercise: A plan to put you in motion
- Medication Counseling: Help with your medication, whether it's pills
or insulin
- Blood Glucose Monitoring: A way to make sure your treatment plan is working
- Overall Diabetes Management: Helping to learn all about your diabetes
- Certified Insulin Pump Instruction Center: Personalized, intensive pump training
What About Educational Sessions?
With Diabetes Self-Management Services, patients receive individualized
one-to-one counseling on diabetes self-management and meal planning. In
addition, an educational group session is offered to expand diabetes knowledge
on healthy eating, being active, monitoring health status, taking medications,
problem solving, reducing health risks, and healthy coping. As patients
complete their educational sessions, they are seen for follow-up visits
to evaluate their diabetes management, the achievement of behavioral goals,
and to modify their treatment plan as needed.
How Do I Enter the Program?
To enter the program, a patient must receive a referral from his or her
physician. The patient then meets with Thibodaux Regional's diabetes
nurse educator or registered dietitian for an initial interview. During
this interview, the nurse or dietitian will identify any special learning
needs the patient may have, as well as formulate the patient's individualized
educational plan depending on his or her specific needs.
ADA National Recognition
Diabetes Self-Management Services of Thibodaux Regional Wellness Education
Center is nationally recognized by the American Diabetes Association (ADA).
The ADA Education Recognition effort is a voluntary process which assures
that approved education programs have met the National Standards for Diabetes
Self-Management Education Programs. Programs that achieve recognition
status have a staff of knowledgeable health professionals who can provide
state-of-the-art information about diabetes management for participants.
